Admission Conditions

Mandatory conditions for admission 


Admissions are scheduled through the attending physician, family doctor, phone, fax, email, or online booking.


DOCUMENTS REQUIRED FOR ADMISSION


1. Identity card or ID document.


2. Referral letter from the family doctor, specialist from outpatient units, or hospital doctor contracted with health insurance houses.


3. Proof of insurance:


a) For employees: certificate issued by the employer (for insured persons from other counties, it is mandatory to have the visa of the health insurance house where they are registered). If medical leave is required, the certificate must state the number of days of medical leave taken in the last 12 months.


b) For authorized individuals: proof of last payment to FNUASS.


c) Proof of payment of contributions on taxable income (dividends or management allowances set by the General Assembly) for associates or administrators of companies.


d) Last pension slip or copy for pensioners.


e) Document attesting the insured status of the person supporting co-insured persons.


f) For unemployment benefit recipients: unemployment coupon, certificate from AJOFM.


g) For uninsured persons required to insure: last payment receipt stating the last month or quarter paid.


h) Certificate from the town hall for social aid beneficiaries, proving eligibility according to Law 416/2001.


i) Passport, European Health Insurance Card for foreign citizens residing outside Romania.


j) Copy of birth certificate for minors, copy of parent's ID.


3. Medical documents such as medical letters, discharge notes, recent laboratory, radiology, and imaging tests.
