Admission Criteria
Patients are admitted based on the following criteria and documents:
- Identity card;
- Admission ticket signed by the specialist or family doctor (within the validity period required by CNAS);
- Health card or replacement certificate from CJAS;
- Proof of insurance (certificate from the workplace, certificate from CJAS, or pension coupon);
- If the patient cannot provide proof of insurance, a payment commitment must be signed;
- For non-reimbursed medical services, a payment commitment for the requested services must be signed;
- Before admission, the patient must take a RT-PCR or Antigen Covid19 test, at their own expense;
Discharge Information
Discharge is made at the recommendation of the attending physician, according to the patient's health evolution.
- Discharge scheduling and communication is done at least 12 hours in advance;
- For patients needing assistance, relatives are notified at least 48 hours in advance;
- Upon discharge, the patient receives: discharge letter/medical letter (at least 2 copies), copy of medical expenses, copy of pathology results (if applicable), medical certificate (if requested), prescription, referral to other specialists or for investigations, medical report for expertise services (if applicable).
Monitoring of Discharged Patients
- Patients with chronic conditions are registered in the admission register;
- Discharge letter mentions follow-up in the outpatient clinic;
- Medical letter for the family doctor with the chronic condition and treatment plan;
- Registration in the chronic disease register and monitoring is required;
- Patients must present to the family doctor with the documents issued by the hospital;
The patient will receive written information about:
- Their condition and possible complications;
- Treatment plan;
- Obligation to follow treatment and hygiene-dietary recommendations;
- Urgent presentation to the doctor for any worsening of the disease;
Monitoring and evaluation after discharge:
- Regular check-ups at the specialty outpatient clinic;
- Regular admissions and reassessment with laboratory tests for therapeutic adjustment;
- Compliance with the therapeutic and lifestyle plan;
