Discharge Information

The attending physician will inform the patient of the discharge date at least 24 hours in advance.

Upon discharge, the patient receives the following documents:

• Discharge summary;
• Medical letter;
• Prescription;
• Diet and lifestyle recommendations;
• Home treatment recommendations;
• Follow-up after a set period;
• Medical leave certificate;

Home medical care is recommended (if necessary), services that can be provided by home care providers.
