About mastectomy with reconstruction

Mastectomy is the surgical removal of one or both breasts, used to treat breast cancer or to prevent it in people with a high genetic risk. At Centrul Medical Topmed in Târgu Mureș, we approach this procedure with professionalism and care, often combining it with breast reconstruction options. This intervention not only treats the disease but also helps restore the patient’s self-esteem, offering both aesthetic and psychological benefits.

When is mastectomy necessary?

Mastectomy can be unilateral (removal of one breast) or bilateral (removal of both breasts). The decision depends on the stage of the disease, the patient's medical history, and personal preferences.

1. Breast cancer
  • In advanced stages, when conservative treatments are insufficient.
  • When the tumor is large relative to the breast size.
  • In cases of recurrent cancer after previous treatments.
2. Ductal carcinoma in situ (DCIS)

An early-stage cancer limited to the milk ducts that can progress to invasive malignancy.

3. Increased genetic risk
  • Patients carrying BRCA1 or BRCA2 gene mutations.
  • Significant family history of breast or ovarian cancer.
4. Benign conditions with high malignant potential

Precancerous lesions such as atypical ductal or lobular hyperplasia.

5. Complications of other treatments

Local recurrence after radiotherapy or incomplete mastectomy.

Types of mastectomy

At Centrul Medical Topmed, procedures are personalized for each patient, taking into account medical condition and personal wishes.

1. Simple (total) mastectomy

Removal of the entire breast tissue, including nipple and areola, while preserving the pectoral muscles.

2. Modified radical mastectomy

In addition to breast removal, affected axillary lymph nodes are also removed to prevent cancer spread.

3. Skin-sparing mastectomy

Removes breast tissue but preserves the skin, facilitating later reconstruction.

4. Prophylactic bilateral mastectomy

Performed to prevent cancer in patients with increased genetic risk, often accompanied by immediate reconstruction.

Breast reconstruction – Restoring confidence and femininity

A key element of modern breast cancer treatment is the option of reconstruction. This can be performed during mastectomy (immediate reconstruction) or after oncological treatments are completed (delayed reconstruction).

1. Breast implants

  • Silicone or saline prostheses restore breast volume and shape.
  • A fast and ideal method for patients without significant tissue damage.

2. Autologous tissue reconstruction (flaps)

  • Tissue is taken from other body areas, such as the abdomen (DIEP/TRAM flap) or back (latissimus dorsi flap).
  • Indicated when local tissue is insufficient or radiation has damaged the skin.

3. Hybrid techniques

Combination of implants and autologous tissue for natural, lasting results.

BRAXON dermal support

To improve implant stability and reduce complications, we use the BRAXON acellular dermal matrix, specially processed from porcine skin.

Additional support

Reduces pressure on surrounding tissues, aiding better healing.

Lower complication rates

Decreases risk of capsular contracture and other implant-related problems.

Natural aesthetic outcomes

Creates a harmonious breast contour and smooth transition between implant and surrounding skin.

This advanced technique offers durable results, comfort, and renewed confidence for patients.

Advantages of breast reconstruction

1
Aesthetic

Restores breast shape and symmetry, supporting a positive body image.

2
Psychological

Enhances self-esteem and reduces the emotional impact of breast loss.

3
Functional

Allows patients to wear normal clothing without external prostheses.

How does the procedure work?

1. Initial consultation

Detailed assessment of medical history, imaging investigations, and patient preferences. Discussions about the type of mastectomy, reconstruction options, and postoperative plan.

2. Surgical procedure

Performed under general anesthesia, lasting 2–4 hours depending on complexity. In immediate reconstruction, the plastic surgeon works as a team with the oncologic surgeon.

3. Postoperative care

Patient is monitored for signs of complications. Detailed instructions are provided for recovery and follow-up appointments.

Recovery and postoperative support

Recovery and postoperative support

Recovery time varies depending on the complexity of the procedure, reconstruction type, and the patient’s overall health:

1. Avoid physical exertion
Intense activities should be avoided for 4–6 weeks.
2. Pain management
Doctor-prescribed analgesics help relieve discomfort.
3. Wound care
Follow instructions for dressing changes and cleaning the surgical site.
4. Monitoring for signs
Notify the doctor if fever, severe pain, or excessive redness occurs.
